Position Overview

This position is critical in enhancing collaborative healthcare delivery across multiple service providers.

The successful candidate will spearhead the advancement of integrated operational frameworks that cater to the health and wellness of diverse communities. This role emphasizes the creation of innovative care models and the effective transformation of service delivery mechanisms.

The position is expected to evolve, contributing to the establishment of a cohesive Community Care Collaborative, facilitating the development and implementation of a unified Locality Operating Model.

Key Responsibilities

- Provide transformational leadership across various Localities, fostering collaborative relationships between primary care, community healthcare, local authorities, and other service providers.

- Exhibit strong, inclusive, and compassionate leadership across the Localities.

- Act as a deputy for the Associate Director of Transformation on designated transformational initiatives.

- Oversee the Locality Managers' operations, ensuring effective delivery strategies are in place for all Localities under the purview of the role.

- Establish systems to monitor and evaluate the performance and activities of all Localities within the designated portfolio.

- Collaborate with Locality Managers to cultivate a culture of accountability and teamwork, ensuring robust communication channels exist for information dissemination among teams and stakeholders.

Responsibilities of the Role

- Lead the Locality Managers in forming effective partnerships, coordinating with multiple organizations to implement Community Care Collaborative initiatives.

- Serve as the escalation point for Locality Integration Managers, providing senior oversight to enhance Locality Partnerships.

- Support operational teams in revising demand and capacity assessments across various services within the Community Care Collaborative framework.

- Collaborate with senior colleagues to develop reporting mechanisms at the Locality level, ensuring alignment with Locality Delivery Partnerships.

- Coordinate the planning and execution of Community Care Collaborative initiatives, articulating the benefits for partners and community members.

- Ensure the development of an annual plan for all Localities that aligns with broader Community Care Collaborative and Integrated Care System (ICS) priorities.

- Maintain a governance framework for planning, reporting, and supporting the three Localities within the designated portfolio.

- Foster partnerships with NHS service providers and other organizations to enhance service delivery and develop business cases for new services as needed.

- Engage in continuous improvement initiatives, advocating for a culture of enhancement within locality partnerships.

- Prepare comprehensive reports summarizing project statuses, outcomes, and progress for senior stakeholders.

- Analyze complex data related to health outcomes to inform decision-making and present findings to relevant committees.

Qualifications and Experience

- Professional qualifications at degree level, supplemented by relevant training and experience.

- Extensive experience in senior management roles, with a strong background in budget management and performance monitoring.
