Support Worker

We are seeking a dedicated and compassionate Support Worker to join our team in the West Kent locality. As a Support Worker, you will play a vital role in supporting individuals with learning and physical disabilities to access and participate in their local community, while also developing essential life skills.
Key Responsibilities:
- Support individuals with learning and physical disabilities to achieve their goals and outcomes
- Plan and deliver outcome-focused sessions to promote social inclusion, rights, and choices
- Assist with personal care and mobility needs, including manual handling
- Work collaboratively with senior staff and other professionals to ensure high-quality care and support
- Drive and escort individuals on the service minibus, requiring a full clean driving license
Requirements:
- Relevant experience in care and support of individuals, including those with learning disabilities
- Awareness of the needs of people with learning disabilities, challenging behavior, autism, epilepsy, and other disabilities
- Diploma Level 2 in Health and Social Care or equivalent, or willingness to undertake training
- Full UK driving license and ability to drive and escort individuals
