The Salvation Army is seeking a highly skilled and compassionate Care Coordinator to join our team at Howard House, a 17-bed Assessment Hub. As a key member of our multi-agency approach, you will work collaboratively to support residents in progressing through the Isle of Wight Homelessness Pathway.
Key Responsibilities:
The Care Coordinator role is instrumental in achieving successful outcomes for our clients. You will be responsible for developing and delivering high-quality, trauma-informed programs of support, promoting clients' strengths, independence, and choice. You will address client support needs, enable them to achieve positive life changes, and ultimately empower them to lead fulfilling lives.
The successful candidate will be able to demonstrate:
- A strong ability to work well in a team environment
- A positive attitude and high level of motivation towards supporting clients to achieve focused and positive life outcomes
- Excellent verbal and written communication skills
- Strong problem-solving skills and ability to manage time effectively
