About the Role

We are seeking an experienced and motivated Nursing Sister/ Charge Nurse/ District Nurse to join our team at East Coast Community Healthcare CIC. As a key member of our multidisciplinary team, you will be responsible for providing clinical leadership, operational management, and professional support to community nursing within our Primary Care Home.

Key Responsibilities
  • Provide clinical leadership and operational management to community nursing teams
  • Work collaboratively with other members of the multidisciplinary team to provide planned and unplanned care
  • Assess and manage patients with complex needs, including those with frailty and complex social issues
  • Develop and implement individualized care plans, utilizing a holistic approach to patient care
  • Provide teaching, training, and supervision to junior staff members
  • Participate in clinical governance and risk management activities
  • Contribute to the development and implementation of service improvement initiatives
Requirements
  • Professional registration with the Nursing and Midwifery Council (NMC)
  • Experience working within a community or intermediate care setting
  • Leadership and management qualifications or willingness to undertake
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Flexibility to work across different sites and travel to alternative sites as required
