Team Leader - Home Care at Specialist Care Team

We are seeking a highly skilled and dedicated Team Leader to join our Home Care team at Specialist Care Team. As a Team Leader, you will play a vital role in ensuring the highest standard of care for our service users.

Key Responsibilities:

  • Manage a caseload of service users, ensuring their needs are met and their care plans are up to date.
  • Lead and support a team of care staff, providing guidance and supervision to ensure excellent care delivery.
  • Develop and implement care plans that promote independence, choice, and well-being for service users.
  • Collaborate with other teams and stakeholders to ensure seamless care delivery and effective communication.
  • Conduct regular assessments and reviews to ensure service users' needs are being met.
  • Provide leadership and support to front-line care staff through regular supervision and appraisal.

Requirements:

  • Proven experience in a care leadership role, with a strong understanding of care principles and practices.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Driving license and access to a car, with the ability to travel to various locations.
