About the Role

The Kent and Medway NHS & Social Care Partnership Trust is seeking a skilled Community Psychiatric Nurse to join their Home Treatment Team. This team provides a responsive assessment and home treatment service to individuals experiencing an acute mental health crisis in the community.

Main Responsibilities

  • Assess and provide home treatment services to individuals in crisis, working collaboratively with service users, carers, and other professionals.
  • Work as a member of the Home Treatment Team and Rapid Response team, providing intensive home treatment and support to patients and their families.
  • Conduct emergency assessments and gatekeepings for patients requiring a 4-hour emergency response.
  • Work in accordance with Trust and Home Treatment Team policies and procedures, as well as the Care Programme Approach.
