Job Summary:

We are seeking an experienced Occupational Therapist to join our Community Independence Service (CIS) team as a Home First and Urgent Community Response Therapist. This is an exciting opportunity for a Band 5 or 6 OT to develop their skills and work in a supportive and friendly team.

About the Role:

As a Home First and Urgent Community Response Therapist, you will work in an integrated way with a multidisciplinary team to provide high-quality, client-led and goal-focused treatment to patients in their homes. You will be responsible for assessing the health and social care needs of patients and planning community services required to support their recovery.

Key Responsibilities:

  • Assess the health and social care needs of patients and plan community services required to support their recovery.
  • Work in partnership with GPs and other healthcare professionals to plan and provide the best care for patients in the community.
  • Support and enable the process of safe discharge from secondary to Community Services and assist in bridging the gap from hospital to home.
  • Proactively promote a seamless and multidisciplinary approach to health and social care provision.
  • Identify and case find potential CIS patients in partnership with acute colleagues and ensure that as soon as patients are medically stable, they are transferred to the appropriate community service.

Requirements:

  • Excellent communication skills
  • Strong clinical knowledge and assessment skills
  • Current HCPC registration
