Mental Health Practitioner

We are seeking an experienced mental health professional to join our multidisciplinary team as a Band 6 Community Mental Health Practitioner. This is an exciting opportunity to work with patients over the age of 18, focusing on OPMH patients with acute mental health concerns or worsening of pre-existing conditions.
Main Responsibilities- Conduct comprehensive, holistic mental health assessments to identify patient needs and develop personalized care plans.
- Collaborate with acute physical health colleagues to deliver integrated, patient-centered care, promoting parity of esteem within the acute setting.
- Provide educational sessions to physical health colleagues to enhance mental health awareness and improve patient outcomes.
As a registered professional, you will become a member of our Academy of Clinical Excellence, focusing on professional development and career progression. Our development pathway for Band 6's enhances your competence and confidence as a senior clinician and leader.

We offer a dynamic work environment, flexible working arrangements, and a competitive salary of £35,392 to £42,618 per annum based on full-time hours.
