Job Summary:
We are seeking a highly skilled and experienced Occupational Therapist to join our Community Falls Prevention Service as a Specialist Practitioner. The successful candidate will work in a multidisciplinary team to provide specialist assessment and rehabilitation services to patients at risk of falls in community and outpatient settings.
Key Responsibilities:
- Deliver high-quality occupational therapy services to patients with complex needs, utilizing evidence-based practice and clinical expertise.
- Develop and implement individualized treatment plans to promote patient independence and occupational wellbeing.
- Collaborate with multidisciplinary teams, including Consultant Geriatricians, Nurses, Physiotherapists, and Technical Instructors, to ensure comprehensive care and support.
- Contribute to the development and implementation of service policies and procedures, ensuring adherence to professional standards and best practices.
- Participate in quality improvement initiatives and contribute to the evaluation and development of service outcomes.
Requirements:
- Postgraduate qualification in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
- Proven experience working in community settings, with a focus on falls prevention and rehabilitation.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to work flexibly and adapt to changing service needs and priorities.
