Position Overview

As a Community Geriatric Care Consultant, you will operate independently within the Frailty Team and collaborate with Primary Care Networks to deliver exceptional patient-focused clinical services. Utilize your clinical expertise, sound judgment, and decision-making abilities to conduct assessments and examinations, providing accurate diagnoses and developing both reactive and proactive management strategies within an established scope of practice.

Assess investigations and modify treatment plans based on patient requirements while evaluating the impact and outcomes of care.

Engage in advanced clinical assessments and examinations, including but not limited to:

  • Interpretation of clinical data, investigations, and treatment plans.
  • Development of comprehensive care plans and evaluation of interventions.
  • Expert diagnosis and management of diseases, delivering specialized clinical interventions and treatments.
  • Non-medical prescribing capabilities.
  • Assessment and treatment of diverse clinical cases, demonstrating advanced skills in evaluating patients with both clear and ambiguous clinical diagnoses.
  • Collaborative work within a multi-professional team, contributing to the education and development of nursing, medical, and allied health professionals.
  • Leadership in multidisciplinary team initiatives focused on promoting healthy aging and managing frailty.

Key Responsibilities

The Community Geriatric Care Consultant will collaborate with their designated Primary Care Network and the broader Neighbourhood Team to address patient needs effectively.

Employ advanced communication, problem-solving, and decision-making skills to conduct initial assessments and provide ongoing patient care. Operating within established local and national protocols, the Consultant will exercise independent judgment to assess, investigate, diagnose, plan, implement, and evaluate the clinical care and management of community patients.

This position is integral to a team that leads innovative non-medical care models across primary and community settings, including care homes and community teams, in partnership with multi-professional and multi-organizational colleagues. The Consultant will play a vital role in proactive care through timely assessments, referrals, responsive interventions, and care planning.
