About the Role

We are seeking an experienced Mental Health Liaison Specialist to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to individuals in acute crisis.

Key Responsibilities
  • Provide effective liaison and communication with healthcare professionals, families, and carers to ensure seamless care coordination.
  • Conduct rapid assessments of individuals with mental health problems in acute crisis and provide alternative solutions to in-patient admission.
  • Develop and maintain effective working relationships with sector community mental health teams, older people's community teams, and other community-based teams.
  • Ensure that all relevant information regarding client care is communicated to the appropriate personnel, bringing attention to issues of concern regarding practice or workload.
  • Liaise with other professionals in relation to client care, actively participating in care planning and review meetings, and attending case conferences as necessary.
Person Specification

We are looking for an individual with the following qualifications and experience:

  • RMN or Dip/BSc (Hons) in Mental Health Nursing
  • Post-registration experience in acute and community areas for younger or older adults, with evidence of continuing professional development
  • Ability to communicate effectively within a multiprofessional team
  • Confidence to work autonomously in the community and use own initiative, making decisions in complex situations
  • Ability to provide and receive complex, sensitive, or contentious information, where persuasive, motivational, negotiating, training, empathic, or reassuring skills are required
