About the Role

Herts Mind Network is seeking a highly skilled and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in providing advice, information, and holistic support to individuals experiencing mental ill health or requiring support with their mental wellbeing.

Key Responsibilities

  • Provide advice, information, and onward referral to clients who are experiencing mental ill health or need support with their mental wellbeing.
  • Work closely with General Practitioners within the Primary Care Network to ensure seamless support and care for clients.
  • Conduct regular reviews to monitor and ensure the safety and wellbeing of clients.
  • Embed a person-centered and recovery-oriented approach in all aspects of the role, incorporating empathy, warmth, acceptance, authenticity, compassion, and humanity.

Accountabilities

  • Receive referrals from a wide range of referral pathways.
  • Undertake initial contact with clients and complete a holistic needs assessment, utilizing assertive outreach work to achieve initial client contact if required.
  • Provide holistic support, both outreach and in-centre, and advocacy services to clients experiencing mental ill health.
  • Ensure all person-centered risk assessments and support plans are completed as necessary.
  • Liaise with other agencies to ensure clients receive appropriate support and remain engaged with the service.
  • Assist clients in accessing mental health services, drug and alcohol services, housing/tenancy services, and other services according to assessed needs.
  • Establish effective pathways across various agencies to maximize partnership working.

  • Annual leave entitlement of 25 days per year pro rata, rising in increments to a maximum of 29 days after 5 years of employment (plus 8 days Bank Holidays).
