Key Responsibilities

  • Provide one-to-one support to residents living with dementia and mental health conditions, using a person-centered approach to promote their well-being and independence.
  • Manage caseloads effectively, working collaboratively with multidisciplinary teams to ensure seamless care delivery.
  • Utilize cognitive assessments and psychosocial interventions to address residents' needs and concerns.
  • Develop empathic relationships with residents, their families, and carers, providing emotional support and guidance when needed.
  • Stay up-to-date with best practices in dementia and mental health care, applying knowledge to inform care decisions and improve outcomes.

Requirements

  • Knowledge of dementia and mental health conditions, including pharmacological and non-pharmacological treatments.
  • Experience of working in a care setting, preferably with older adults.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to remain motivated and composed in challenging situations, prioritizing resident well-being and safety.
  • Formal qualification in a relevant field, such as health and social care, psychology, or counseling.
