Position Overview
This role is integral to the Community Therapy Support Specialist rotation, delivering therapeutic interventions and care within various settings, including patients' residences, healthcare facilities, and transitional care environments. The position plays a crucial role in the community therapy framework, aimed at minimizing hospital admissions, reducing the duration of hospital stays, and enhancing clinical outcomes upon discharge.
The rotation encompasses teams such as the Community Stroke Team, Supported Discharge Team, and the Urgent Community Response Therapy Team.
The Community Therapy Support Specialist will operate under the guidance of licensed therapy professionals, executing assessment and treatment plans for patients in community settings and transitional care facilities as necessary.
Key Responsibilities
The Community Therapy Support Specialist is equipped to manage a complete care episode from assessment through to discharge for a defined clinical caseload as designated by a licensed clinician.
The incumbent will assist with administrative and clerical tasks for the team and department.
Core Duties Include:
- Conducting all clinical and care responsibilities under the supervision of a therapist.
- Managing a full patient caseload with oversight from a therapist.
- Assessing and addressing equipment needs for patients.
- Facilitating patient progress in their homes to enhance function and independence.
- Providing education and instruction to family members, caregivers, colleagues, and other professionals to promote understanding of therapy objectives and ensure continuity of care.
- Contributing to educational sessions for relevant patient demographics.
- Participating in multidisciplinary team meetings to coordinate patient care, including progress reviews and discharge planning.
- Utilizing skills such as persuasion and motivation to encourage adherence to treatment plans.
- Maintaining accurate clinical documentation of all interventions.
- Generating patient-related reports for other professionals or agencies regarding assessment outcomes and treatment progress.
- Inputting data into relevant databases as required.
- Collaborating with qualified practitioners to manage equipment provision and rehabilitation from pre-operative to post-operative phases.
- Delivering and setting up basic aids and equipment in patients' homes, instructing patients and caregivers on proper usage.
- Conducting home visits and assessments, with or without a therapist, as needed.
- Managing daily workload effectively to maintain service levels within the local pathway and assisting team members in prioritizing tasks.
Workforce Development
- Providing guidance and training to therapy assistants within the organization.
- Engaging proactively with therapy and nursing staff to relay patient progress and highlight any clinical concerns.
- Participating in supervision and performance evaluations with a designated senior therapist.
- Attending team meetings and contributing to the clinical orientation of new therapy staff.
- Independently advancing patients' exercise programs and mobility, including the provision of walking aids for specified patient groups.
Financial Responsibilities
- Ensuring availability and accessibility of daily living equipment, collaborating with relevant services.
Partnerships and Communication
- Communicating directly with ward staff regarding pertinent patient care information and discharge facilitation.
- Providing information and guidance to other professionals to ensure a cohesive approach to patient care.
- Educating patients, relatives, and caregivers about their conditions and the goals of therapeutic interventions.
General Responsibilities
- Maintaining accurate electronic and written records that comply with professional standards.
- Performing general administrative duties, including filing, appointment scheduling, processing referrals, and answering calls.
- Utilizing a company vehicle for patient home visits as required.
- Exhibiting flexibility to work across community and hospital settings based on demand, while effectively organizing and prioritizing workload.
- Assisting with personal hygiene, continence management, medication administration, light housekeeping, shopping, and meal preparation as part of care duties.
- Willingness to participate in weekend and out-of-hours shifts as necessary.
Qualifications & Training
Essential:
- Evidence of study or attainment of NVQ Level III or equivalent qualifications and experience.
Experience:

- Clinical experience in physiotherapy or occupational therapy services.
- Experience working in community settings.
- Proficiency in IT applications.
Desirable:
- Experience in both community and acute care environments.
- Experience in a caregiving setting.
Knowledge:
Essential:
- Understanding of fundamental rehabilitation principles.
- Ability to utilize computer databases and maintain electronic patient records.
- Capability to supervise junior staff.
- Awareness of care pathways and key issues relevant to the organization.
