About the Role

We are seeking an experienced and skilled Advanced Care Practitioner to join our team at West View Millennium Surgery. As a key member of our primary care team, you will be responsible for delivering high-quality patient care, working autonomously and in collaboration with other healthcare professionals to assess, diagnose, and treat patients.

Key Responsibilities
  • Manage a clinical caseload, dealing with presenting patients' needs in a primary care setting.
  • Lead the implementation of clinical contractual needs of the practice, including Quality and Outcome Framework and Enhanced Services.
  • Provide expert professional advice to patients, carers, and colleagues, ensuring the maintenance of clinical excellence.
  • Develop new and innovative concepts, models, methods, and practices to deliver new and improved care services to meet the needs of service users.
  • Provide education and training to other staff and students.
  • Undertake audit and education as part of the role.
Requirements
  • Experience of working in a primary care environment.
  • Post Graduate Diploma/Degree (Advanced Practice Qualification).
  • Minimum four years' experience in primary care.
  • Demonstrate evidence of study at master's level/master's degree.
  • Clinical skills training/qualification and experience.
