Position: Social Worker - First Response Team
Compensation: £38 per hour (Umbrella)

The First Response Team provides essential short-term assistance to individuals within the community. This role involves close collaboration with the reablement service, Care Coordination Centre Triage Clinicians, and various Community Services. As a Social Worker in this dynamic team, your responsibilities will include conducting assessments in accordance with the Care Act 2014 and the Mental Capacity Act 2005. You will engage directly with clients, their families, and other professionals to ensure comprehensive support.
Key Responsibilities:
- Conduct assessments and develop care plans for adults requiring support.
- Provide short-term case management and facilitate Multi-Disciplinary meetings.
- Complete initial safeguarding inquiries to protect vulnerable individuals.
- Work primarily in the designated region while maintaining flexibility across the area.

Qualifications Required:
- A relevant Social Work qualification.
