About the Role:

We are seeking a qualified and experienced Social Worker to join our Frimley Park Hospital Social Work Team. As a key member of our team, you will work closely with dedicated health and social care colleagues to deliver excellent social work services and support adults leaving hospital to achieve the best possible outcomes.

Key Responsibilities:
  • Conduct comprehensive assessments of individuals' needs, strengths, risks, and available support.
  • Triage safeguarding referrals and make informed decisions.
  • Manage a complex caseload while providing supervision for Case Workers and Senior Case Workers and practice advice on complex cases.
  • Build positive and professional relationships.
Requirements:
  • Social Work qualification and registration with Social Work England (SWE).
  • A sound understanding of how to apply a strengths-based approach.
  • Understanding of social work issues, legal frameworks, and social services' statutory duties.
  • Commitment to demonstrating our values and behaviours.
