Position Overview: We are looking for a dedicated nurse who is committed to excellence in mental health care for older individuals, with a strong interest in service enhancement and quality improvement. The ideal candidate will foster a workplace environment that aligns with our Trust's cultural values, ensuring that both staff and service users feel secure, appreciated, and empowered.


Key Responsibilities:

  • Manage a caseload of older adults requiring health and social care interventions.
  • Conduct home visits, necessitating proficiency in navigating the local area via public transport, walking, or cycling.
  • Collaborate with service users and their families/carers to deliver a range of interventions.
  • Supervise and support other team members.
  • Engage with a multidisciplinary team and maintain close communication with GPs, primary care services, secondary health services, and voluntary organizations.


Qualifications: Candidates should possess experience working with older adults suffering from severe mental illness and/or dementia, alongside exceptional communication and interpersonal skills. A solid understanding of the broader health and social care landscape is essential.


Technical Skills: Proficiency in computer usage is required, as the role involves working with Electronic Patient Records and various software applications, including MS Word and Teams.
