About Us: We are the Islington Services for Ageing and Mental Health (SAMH) Community Mental Health Team (CMHT), a premier health and social care provider in the London Borough of Islington, delivered by the Camden & Islington NHS Foundation Trust. Our mission is to support older adults facing mental health challenges and those impacted by dementia.
Position Overview: We are looking for a dedicated nurse who is committed to excellence in mental health care for older individuals, with a strong interest in service enhancement and quality improvement. The ideal candidate will foster a workplace environment that aligns with our Trust's cultural values, ensuring that both staff and service users feel secure, appreciated, and empowered.
Key Responsibilities:
- Manage a caseload of older adults requiring health and social care interventions.
- Conduct home visits, necessitating proficiency in navigating the local area via public transport, walking, or cycling.
- Collaborate with service users and their families/carers to deliver a range of interventions.
- Supervise and support other team members.
- Engage with a multidisciplinary team and maintain close communication with GPs, primary care services, secondary health services, and voluntary organizations.
Qualifications: Candidates should possess experience working with older adults suffering from severe mental illness and/or dementia, alongside exceptional communication and interpersonal skills. A solid understanding of the broader health and social care landscape is essential.
Technical Skills: Proficiency in computer usage is required, as the role involves working with Electronic Patient Records and various software applications, including MS Word and Teams.
