About the Role

We are seeking a skilled and compassionate Community Support Worker to join our team at Midlands Partnership NHS Foundation Trust. As a Community Support Worker, you will play a vital role in providing support and therapeutic time to individuals with mental health needs, promoting their recovery and helping them maintain their place in the community.

Key Responsibilities

  • Provide support and spend therapeutic time with allocated service users, promoting their recovery and assisting them to maintain their place in the community.
  • Work in a multi-disciplinary team and in partnership with internal and external agencies to assess, plan, implement, and evaluate individualised care plans.
  • Assist the Shift Co-ordinator/Lead Professional in planning, implementing, and evaluating individualised care plans.

Qualifications

  • Advanced Apprenticeship in (Mental) Health and Social Care or NVQ 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills)

Experience

  • Experience of being a service user or carer
