Hospice Palliative Care Nursing Assistant

We are seeking a skilled and compassionate Palliative Care Nursing Assistant to join our team at Rossendale Hospice. As a key member of our care team, you will provide high-quality nursing care to patients and their families, working closely with our multidisciplinary team to deliver evidence-based care.
Key Responsibilities- Provide skilled nursing care to patients in their homes and in our Wellbeing setting, under the direction of the Team Leader.
- Manage Wellbeing therapy sessions and co-ordinate daily activities with the Hospice at Home service.
- Undertake patient assessments and contribute to planning, implementation, and evaluation of programmes of care for individual patients and their families.
- Liaise with the multidisciplinary team, including Palliative Consultant, District Nurse, Social Workers, GPs, and other agencies involved in patient care.
- Support people's equality, diversity, and rights, and maintain and support the efficient use of physical and/or financial resources.
- Assist with the supervision and support of Health Care Assistants, health care students, and volunteers.
- Take part in duty rotas to ensure nursing services are covered and contribute to the monitoring and evaluation of clinical services.
- Collect and report data on attendance figures, safe staffing tools, patient outcomes, and experience.
- Registered with the NMC and adherence to the professional Code of Conduct.
- Ability to work autonomously and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Flexibility to work varied shifts, including evenings and weekends.
