About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at West London NHS Trust. As a key member of our Integrated Care Team, you will play a vital role in providing high-quality mental health support to individuals in our community.

Key Responsibilities
  • Provide holistic mental health support to individuals, taking into account their physical and social needs.
  • Work closely with primary care networks, social services, and other organizations to provide integrated care and support.
  • Develop and implement person-centered care plans, working collaboratively with individuals and their families.
  • Conduct risk assessments and provide crisis intervention skills to support individuals in need.
  • Use psychosocial assessment methods to inform treatment and care plans.
  • Communicate effectively with individuals, families, and other healthcare professionals to ensure seamless care.
Requirements
  • Professional qualification in mental health, such as Mental Health Nursing (RMN) with current NMC registration or registration with Social Work England for qualified social workers.
  • Experience of post-qualifying work in psychiatry and supervising junior staff.
  • Knowledge of counseling skills, cognitive behavioral skills, and psychosocial interventions.
  • Ability to interpret research and apply it to practice.
  • Specialist knowledge of mental health conditions and their relationship to physical disorders.
