About the Role

We are seeking a highly skilled and dedicated Social Worker to join our Adult Care Team at Worcestershire County Council. As a Social Worker, you will play a vital role in supporting adults with physical disabilities and mental/physical ill health to live independently and achieve their goals.

Key Responsibilities
  • Conduct comprehensive assessments to identify the needs and strengths of adults requiring care and support.
  • Develop and implement personalized care plans to promote independence, well-being, and quality of life.
  • Work collaboratively with health colleagues, statutory partners, and voluntary sector providers to ensure seamless service delivery.
  • Advocate for adults to access universal services and support packages, promoting autonomy and creative working styles.
  • Manage a caseload, providing regular supervision and support to ensure high-quality practice.
Requirements
  • Strong assessment, planning, and intervention skills, with experience in complex cases and situations.
  • Extensive knowledge of the Care Act and Mental Capacity Act, with ability to apply this knowledge in practice.
  • Ability to work autonomously, with high level of skill in self-assessment and self-approval of support plans.
  • Valid driving license and access to own vehicle.
  • Commitment to a strength-based approach, promoting positive changes in the lives of adults.
