The successful candidate will be responsible for prioritizing, allocating, and reviewing work, ensuring effective budget management and maximizing service efficiencies.

Key Responsibilities:
  • Support Senior Colleagues: Collaborate with senior team members to prioritize, allocate, and review work, ensuring adherence to policies, procedures, and statutory requirements.
  • Service Monitoring and Evaluation: Contribute to service monitoring and evaluation, undertaking needs analysis, reviewing resource availability, and developing service practice, policies, and strategies to maintain high-quality services.
  • Supervision and Guidance: Provide supervision, direction, and coordination of workloads, support, and guidance to team members, as well as appraisal and professional development.
  • Service User Assessments: Undertake clear and detailed service user, carer assessments, and interventions in accordance with relevant statutory requirements, minimizing risks and considering service user needs and available options.
  • Complex Case Management: Manage high-complex cases, including planning and review, promoting independence and early intervention, and minimizing risks and improving quality of life for service users and their carers.
  • Community Engagement: Work with individuals, families, carers, and communities to help them make informed decisions, enabling them to clarify their needs, construct value-for-money care packages, and commission appropriate services.
  • Statutory Responsibilities: Assist in ensuring that statutory responsibilities are met and fulfilled by the team, ensuring quality practice standards are achieved in delivering high-quality services.
  • Preventative Work: Undertake preventative work and risk assessments, utilizing community resources and services such as reablement and telehealthcare.
  • Transition Management: Manage complex transitions cases and chair multi-disciplinary team meetings as appropriate to specialized field/local structures.
  • Professional Development: Maintain up-to-date knowledge of best practice, policy, and procedure in relation to vulnerable people, as well as keeping abreast of wider service developments to enhance the service accordingly.

Requirements: The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. They will also have a strong understanding of safeguarding principles and practices, as well as experience in working with vulnerable individuals.
