Key Responsibilities

The following are the primary duties expected of the Healthcare Practitioner. Additional tasks may be assigned based on workload and staffing needs:

a. Ensure compliance with all cytology programs and adhere to national guidelines.

b. Actively participate in the nursing team to fulfill quality contract requirements, including QOF and GP specifications.

c. Assess patient needs to provide appropriate healthcare services, utilizing evidence-based practices.

d. Develop, implement, and integrate health promotion and wellness initiatives.

e. Formulate and evaluate personalized treatment plans for patients with chronic diseases, promoting overall health and wellbeing.

f. Identify and support patients at risk of developing long-term health conditions to prevent adverse health outcomes.

g. Provide routine nursing care in accordance with clinical evidence, NICE guidelines, and the NSF as necessary.

h. Diagnose and manage both acute and chronic health conditions as required.

i. Review and optimize patient medication regimens to enhance adherence.

j. Offer guidance to patients regarding prescribed and over-the-counter medications, including potential side effects and proper dosages.

k. Coordinate travel medicine services for patients.

l. Request pathology services and manage results as needed.

m. Participate in chronic disease clinics, delivering necessary patient care and referring to secondary or specialist services when appropriate.

n. Maintain accurate clinical records in compliance with existing legislation.

o. Ensure effective use of Snomed codes by all nursing team members.

p. Oversee the maintenance and utilization of chronic disease registers.

q. Engage in well-woman clinics and baby immunization sessions.

r. Adhere to the practice chaperone policy.

s. Support winter vaccination initiatives.

t. Prioritize health issues and intervene as necessary.

u. Assist the team in managing clinical emergencies.

v. Recognize, assess, and refer patients with mental health needs.

w. Contribute to vaccination programs for both adults and children.

Additional Responsibilities

Beyond the primary responsibilities, the Healthcare Practitioner may be called upon to:

a. Review and enhance audit protocols and processes within the practice, effectively utilizing the audit cycle.

b. Support all nursing team members in their roles.

c. Engage in local initiatives aimed at improving service delivery and patient care.

d. Foster shared learning within the practice to enhance patient care.

e. Continuously assess clinical practices in response to national policies and initiatives.

f. Participate in the review of significant and near-miss events using structured approaches, including root cause analysis (RCA).

Essential

- Registered Nurse (Nursing and Midwifery Council)

- Mentor or teaching qualification

- Postgraduate diploma or degree in Chronic Disease Management

- Nurse prescriber

- Completed or in progress formal infection prevention and control training

- Family planning qualification

Experience

- Experience in a primary care setting

- Participation in a nursing team within a primary care environment

- Chronic disease management experience

- Ability to work autonomously

- Experience as a practice nurse or community nurse

- Knowledge of infection prevention and control measures

- Experience managing elderly and housebound patients

- Familiarity with quality initiatives and benchmarking
