Position Overview

We are pleased to announce a part-time opportunity for a Band 7 Dietitian to join our community-focused team. This role is designed to spearhead the execution of a locally established malnutrition management strategy, emphasizing training, education, and advocacy. The successful candidate will cultivate collaborative relationships with Primary Care Networks to promote the adoption of best practices in malnutrition and hydration management.

The community dietetic service is comprised of a dynamic and proactive group of dietitians and dietetic assistant practitioners serving various inner boroughs.

Our team is dedicated to empowering patients to meet their nutrition and hydration objectives through evidence-based dietetic care.

We prioritize team cohesion and well-being, meeting in person monthly and virtually weekly. Our team members have access to office facilities across the boroughs, along with the flexibility to work from home.

The post holder will integrate into a broader network of Dietitians within the Trust, with an expectation of participation in Quality Improvement initiatives. We emphasize continuous learning and development, delivering personalized care, and employing a health coaching approach for those utilizing our services. Training and supervision in Health Coaching will be provided.

Key Responsibilities

• Lead the implementation and evaluation of the malnutrition management strategy across community nursing teams.

• Collaborate with prescribing leads to create and execute a training program that aids Primary Care Networks in identifying and managing malnutrition and enhancing hydration practices.

• Foster and develop partnerships with Primary Care Networks to advocate for the adoption of best practices in malnutrition and hydration management.

• Provide expert advice on integrated and holistic care as part of a population health management strategy during multidisciplinary team meetings.

• Drive multidisciplinary and multi-agency collaboration in clinical specialties to inform service development in nutrition support.

Qualifications and Experience

Essential:

  • Degree in Dietetics.
  • Registration with the Health and Care Professions Council.
  • Evidence of post-registration education and training in nutrition support.

Desirable:

  • Membership in the British Dietetics Association.
  • Participation in relevant Special Interest Groups.
  • Training in supervisory skills.
  • Post-registration training in Behaviour Change or Health Coaching.
