Position Overview

The LDA Hospital at Home initiative is a groundbreaking service being established under the Thames Valley Provider Collaborative. This team will concentrate on adolescents with moderate to severe learning disabilities and/or significant functional impairments linked to autism diagnoses, who are at risk of home or placement disruption and may qualify for inpatient treatment.

In certain situations, the team will aid in averting unnecessary hospitalizations, while in others, adolescents will transition from tier 4 care through the LDA Hospital at Home initiative, thereby reducing their time in inpatient settings. By leveraging innovative technology, we provide care close to home, minimize admissions, shorten lengths of stay, reduce waiting times, and facilitate earlier discharges from inpatient care. This approach will:

  • Alleviate the need for families to travel long distances
  • Enhance ongoing engagement with community clinical teams
  • Promote early therapeutic home visits
  • Minimize Length of Stay
  • Prevent admissions

Key Responsibilities

As a Nursing Associate within the LDA team, you will possess a comprehensive understanding of nursing practices, delivering holistic and individualized care and support to young individuals up to 18 years old across various environments. The Nursing Associate operates autonomously under the guidance of Registered Nurses, engaging in all facets of the nursing process.

This role will support registered nursing staff (Band 5 and above) and other healthcare professionals in executing a diverse array of nursing interventions, including the care and management of various mental health conditions, as well as supporting young individuals with learning disabilities and/or autism and social vulnerabilities. It necessitates strong capabilities, insights, and adaptability in both physical and mental health, alongside excellent organizational, communication, and interpersonal skills.

Your primary location will be in the new offices within the Meadow Unit, with planned visits throughout the community across the Thames Valley collaborative area. Flexibility in working hours is required, typically scheduled between 8 am and 8 pm.

Job Responsibilities

Work collaboratively on a shared caseload under the supervision of a registered Nurse. Responsibilities include:

  • Documenting work in electronic patient records
  • Contributing to nursing assessments, including initial assessments and nursing evaluations
  • Participating in the design and execution of clinical interventions, including care planning
  • Monitoring and reporting on the physical and mental well-being of service users, escalating care needs as necessary
  • Coordinating with appropriate health services and community-based support
  • Delivering specific therapeutic interventions under supervision to address the holistic needs of individuals and their families
  • Arranging and attending patient and carer monitoring visits as part of a planned care package
  • Communicating care plans to patients and families, utilizing accessible information
  • Updating care plans and electronic records under the guidance of a registered Nurse
  • Conducting health education regarding the impact of mental health on overall well-being
  • Monitoring medication adherence and educating patients on potential side effects
  • Maintaining accurate records and documentation
  • Engaging in multi-disciplinary teamwork to deliver comprehensive, evidence-based assessments and interventions

Knowledge and Skills Requirements

Essential

  • Understanding of when to seek guidance and refer to a registered health or care professional
  • Ability to recognize personal limitations within the nursing associate role
  • Awareness of the importance of adhering to procedures and treatment plans
  • Capability to support and mentor nursing associate students and other learners within the clinical environment

Experience

Essential

  • Prior experience working with young individuals or adults with learning disabilities or autism

Desirable

  • Significant experience in a relevant health or care setting
  • Previous experience in a larger organization and within a multi-disciplinary team
