The LDA Hospital at Home initiative is a groundbreaking service being established under the Thames Valley Provider Collaborative. This team will focus on supporting young individuals with moderate to severe learning disabilities and/or significant functional impairments linked to autism diagnoses, who are at risk of home or placement disruption and may qualify for inpatient treatment.

In certain situations, the team will help avert unnecessary hospital admissions, while in others, young individuals will transition from tier 4 care through the LDA Hospital at Home team, thereby reducing their inpatient duration. This service leverages innovative technology, delivers care close to home, minimizes admissions, shortens lengths of stay, reduces waiting times, and facilitates earlier discharges from inpatient settings.

By managing the care pathway, we ensure that children and young people receive care in either an inpatient environment or through an 'out of hospital' service as close to home as possible. This approach will:

  • Alleviate the strain on families from traveling long distances
  • Enhance ongoing engagement with community clinical teams
  • Promote early therapeutic home leave
  • Minimize Length of Stay
  • Avoid unnecessary admissions

Main Responsibilities

As the LDA Healthcare Support Associate, you will possess a comprehensive understanding across various nursing fields, delivering holistic and person-centered care and support for young individuals up to 18 years old in diverse environments. You will operate independently under the guidance of Registered Nurses, engaging in all facets of the nursing process.

This role will assist registered nursing staff (Band 5 and above) and other healthcare professionals in executing a wide array of nursing interventions, including the care and treatment of various mental health conditions, as well as supporting young individuals with learning disabilities and/or autism and social vulnerabilities. It demands strong capabilities, insights, and adaptability in both physical and mental health, alongside organizational, communication, and interpersonal skills.

Your primary location will be in the new offices within the Meadow Unit, with planned visits throughout the community across the Thames Valley collaborative area. Flexibility in working hours is required, typically scheduled between 8am and 8pm.
