Key Responsibilities
- Work collaboratively with individuals, their families, and healthcare professionals to develop personalized support plans that promote independence and well-being.
- Provide practical support and guidance to individuals with dementia, enabling them to maintain their daily living skills and confidence.
- Contribute to the development and delivery of community-based support programs that promote social inclusion and community engagement.
- Work closely with other healthcare professionals to prevent avoidable hospital admissions and ensure seamless care transitions.
- Provide crisis support and emergency response services to individuals with dementia, ensuring their safety and well-being at all times.
Requirements
- Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and healthcare professionals.
