This position presents a unique opportunity to facilitate patient transitions from acute care, collaborating with a skilled team to ensure optimal outcomes.
The primary objective is to foster a cooperative approach to patient discharge, ensuring that individuals receive care for the necessary duration of their acute treatment and experience a smooth transition to subsequent care environments when required.
The role necessitates comprehensive assessment capabilities to identify patient needs holistically and to assist in establishing realistic discharge goals in conjunction with the multidisciplinary team (MDT).
Key Responsibilities
- Conduct timely and effective assessments of patient needs.
- Serve as a liaison among various agencies and departments to ensure efficient communication and expedite the discharge process.
- Cultivate collaborative relationships with ward teams to enhance patient flow across the healthcare system.
- Work in partnership with clinical teams and health and social care organizations to manage patient flow through acute care facilities.
- Participate in follow-up assessments, care planning, resource identification, and service delivery to facilitate appropriate and timely patient discharges.
- Demonstrate flexibility and proactivity in addressing service needs, working across organizational boundaries.
- Utilize advanced communication skills effectively in complex and potentially challenging situations.
- Provide guidance, education, and support to colleagues as necessary.
- Advocate for patients and their families or caregivers.
- Take an active role in resolving minor conflicts, collaborating with senior staff as needed.

Qualifications and Skills
Essential Requirements
- Current professional registration with the Nursing and Midwifery Council (NMC).
- Professional qualifications as a Registered Nurse (RN) or Registered Mental Nurse (RMN).
- Postgraduate education in relevant areas such as Alert or Long-term Conditions.
- Relevant post-registration experience.
