An vacancy has arisen for an enthusiastic Physiotherapist to join the Homefirst East Team. This position would be an excellent opportunity for a Physiotherapist with experience of working in the community or hospital settings to develop their assessment and rehabilitation skills.
You will be supported to develop whilst being part of a dynamic, progressive, multi-disciplinary team. The Team in East-Cornwall work closely with partnership organisations to support the intermediate health needs of our local population. The team are key to supporting discharges from hospital settings and preventing admission to hospital. You will be responsible for the assessment, planning and implementation of reablement and care. You will assist in the interpretation of relevant information, identifying problems and needs of a person and setting realistic goals. You will have excellent organisational skills combined with a wide knowledge base and experience of working with older persons, the community or hospital services.

To provide a high standard of Therapy Skills to the service, performing specialist assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment and/or rehabilitation plan.
To operate as a specialist practitioner being first contact for urgent requests to help both prevent hospital admission and facilitate early discharge.
To utilise specialist skills in community assessment and management of the acutely sick patient.
To provide leadership for support staff and junior staff through supervision, training and appraisal.
To participate in the planning and development of services within the designated team.
Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.
