Community Nurse

We are seeking a highly motivated and experienced Community Staff Nurse to join our dynamic team at Southern Health NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in delivering high-quality care to patients in their homes and community settings.
As a member of our team, you will work closely with multidisciplinary teams, including GPs, local hospices, and specialist nurses, to provide holistic care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities:
- Plan and deliver person-centred care to patients in their homes and community settings
- Work collaboratively with multidisciplinary teams to coordinate care and services
- Develop and maintain strong relationships with patients, families, and carers
- Contribute to the development and implementation of care plans that meet the needs of patients
- Participate in quality improvement initiatives to enhance patient care and outcomes
Person Specification
We are looking for a candidate with:
- Registration with the NMC as a RGN
- Experience of planning and delivering person-centred care
- Ability to work collaboratively with multidisciplinary teams
- Strong communication and interpersonal skills
- Ability to manage and prioritise workload
