Community Health Nurse

We are seeking a highly motivated and enthusiastic Band 5 Community Staff Nurse to join our dynamic and supportive team. As a Community Staff Nurse, you will play a vital role in providing high-quality care to patients in various community settings, including their homes and clinics.
Our team is committed to delivering person-centred care that addresses the physical, emotional, and social needs of our patients. We work closely with multidisciplinary teams, including GPs, local hospices, complex care nurses, health and social care coordinators, specialist nurses, and therapist colleagues.
This is an excellent opportunity for a nurse looking to start their career in community nursing or for those seeking to continue or progress their community nursing career. You will have the chance to develop skills such as end-of-life care, wound care, and holistic care planning.

We are currently working closely with other NHS Trusts to integrate our community, mental health, and learning disability services. Our new organisation, Hampshire and Isle of Wight NHS Foundation Trust, will launch in July 2024.

Person Specification- Registered General Nurse (RGN) - NMC registered
- Experience of planning person-centred care
- Experience of organising resources relating to patient needs
- Multiprofessional working experience
- Ability to manage and prioritise workload
- Demonstrate initiative in resolving issues and problems
- Car driver and own transport

Salary: £29,970 to £36,483 per annum (based on full-time hours)
