Wakefield Council is seeking an experienced Service Delivery Manager to join our team. As a key member of our commissioning team, you will play a crucial role in shaping the delivery of adult health and social care services across the district.
Key Responsibilities:- Lead the evaluation and monitoring of care home models of care for Wakefield system
- Deliver better outcomes for the population of Wakefield District by leading, influencing, and shaping the joint commissioning of adult health and social care services on behalf of Wakefield Metropolitan District Council (WMDC) and Wakefield Clinical Commissioning Group (WCCG)
- Lead commissioner for adult social care (domiciliary care and care homes) for WMDC and commissioner for care provision for Wakefield Place West Yorkshire ICB
- Shape the market by identifying future need and services, and how best these should be delivered, taking a population-based approach
- Work with the NHS and other partners to integrate commissioning teams and services across a district-wide footprint
- Work with continuing health care, the Chief Nurse, social care staff, and other key stakeholders to improve the quality of domiciliary care and care home services across the District, including developing a Care Homes Joint Quality Assurance Framework across WCCG and WMDC and exploring the introduction of a joint virtual 'quality team'
- Work with Mid Yorkshire Hospital NHS Trust Community Health teams to ensure that they are effectively targeting their support to improve quality for our care home residents and care home settings
- Provide assurance to regulators such as the CQC and NHS England about the quality of the commissioning of care
- Educated to degree level, qualification in a relevant field, NVQ Level 5, or equivalent senior management experience
- Continuing professional development in a relevant field
- We offer a flexible full-time working pattern of 37 hours per week
