As a Community Care Assistant, you will play a crucial role in our innovative approach to care. Your Responsibilities Will Include:
Adapting to flexible working hours and dynamic changes.Providing essential practical and emotional support to individuals in their homes while assessing their needs.Collaborating with individuals, their families, and other professionals to develop personalized care plans.Assisting individuals with daily living activities, including personal hygiene, meal preparation, medication management, and mobility support.Working alongside healthcare professionals and social workers to ensure a cohesive approach to care.Monitoring and evaluating individuals' progress, making necessary adjustments to care plans to support optimal recovery.Maintaining detailed and accurate documentation of interactions and care plans.
To Succeed in This Role, You Will Need:
Experience in a similar care role, ideally within a healthcare or social care environment.A compassionate and empathetic disposition, with a commitment to positively impacting individuals' lives.Strong communication and interpersonal skills, enabling effective teamwork.Understanding of person-centered care principles and a dedication to preserving dignity and autonomy.Ability to work independently, demonstrating initiative and reliability.Flexibility to work various shifts, including evenings and weekends.A full driving license and access to a vehicle is preferred but not mandatory.
