About the Role:- Support the MedGen Community team in delivering high-quality care packages, ensuring the best possible outcomes for our clients.
- Manage and maintain a caseload of clients, building positive relationships with them and their families or wider networks.
- Thoroughly understand clients' and families' needs, ensuring staff competencies match care plans.
- Supervise and assess staff competencies, providing coaching and guidance to build capability.
- Support investigations following incidents or complaints, identifying and reporting environmental and personal changes that may pose risks.
- Effectively communicate with the Registered Manager and Clinical Lead to ensure good management, quality, and safety within the package.
- Support the wider team to ensure all shifts are covered and provide emergency cover when required.
- RNLD qualification with current NMC registration.
- Minimum 2 years' experience in community care or a similar setting.
- Coaching experience (Train the Trainer qualification preferred).
- Strong understanding of CQC Regulations and Social Care Codes of Practice.
- A proactive team player with excellent relationship-building skills.
- Full UK driving license and willingness to travel within the South West region.
