Job Title: Registered Learning and Disability Nurse

At MCS Healthcare, we're seeking a dedicated and compassionate Registered Learning and Disability Nurse to join our team as a Case Manager - Community Services (RNLD). This exciting opportunity will enable you to make a meaningful difference in the lives of individuals with complex care needs, while shaping the future of community care.

About the Role:
  • Support the MedGen Community team in delivering high-quality care packages, ensuring the best possible outcomes for our clients.
  • Manage and maintain a caseload of clients, building positive relationships with them and their families or wider networks.
  • Thoroughly understand clients' and families' needs, ensuring staff competencies match care plans.
  • Supervise and assess staff competencies, providing coaching and guidance to build capability.
  • Support investigations following incidents or complaints, identifying and reporting environmental and personal changes that may pose risks.
  • Effectively communicate with the Registered Manager and Clinical Lead to ensure good management, quality, and safety within the package.
  • Support the wider team to ensure all shifts are covered and provide emergency cover when required.
Requirements:
  • RNLD qualification with current NMC registration.
  • Minimum 2 years' experience in community care or a similar setting.
  • Coaching experience (Train the Trainer qualification preferred).
  • Strong understanding of CQC Regulations and Social Care Codes of Practice.
  • A proactive team player with excellent relationship-building skills.
  • Full UK driving license and willingness to travel within the South West region.
