Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at our long-term care facility in Claremont, NH. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Responsibilities
  • Observe and assess residents' physical and mental well-being, reporting any changes or concerns to the supervisor.
  • Administer medications and treatments as ordered by the physician, accurately documenting all interactions in the medical record.
  • Supervise Licensed Nursing Assistants (LNAs) and ensure they are providing quality care, adhering to licensing requirements and facility policies.
  • Participate in resident rounds with the physician, addressing medical needs and facilitating communication.
  • Administer oxygen and gastrostomy/jejunostomy feedings as per facility guidelines and procedures.
  • Perform wound care and maintain knowledge of wound vacs.
  • Conduct regular unit duties, including checking appointments, monitoring LNA documentation, and ensuring a clean and hazard-free environment.
  • Admit, discharge, and transfer residents, completing necessary documentation and notifications.
  • Complete 24-hour reports, check new orders, and maintain consistent communication with the Nurse Manager/Nursing Supervisor.
  • Communicate with residents' families to resolve concerns and notify them of new orders.
  • Assume responsibility for the management of assigned units during shifts, under the direction of the Nursing Supervisor or Director of Nursing Services.
  • Follow state and federal regulations regarding controlled medications, including counting at shift change and notifying supervisors of discrepancies.
  • Assist with new nursing employee orientation and communicate with the DON, Nurse Manager, or SDC regarding continuing education needs.
Requirements
  • 3 years of current experience as a LPN, non-negotiable.
  • Must be willing to float.
  • IV certification preferred.
  • Previous travel experience and rural/small facility experience highly preferred.

  • Skills checklist (pertinent to position), current to 1 year.
  • 1 supervisory signed reference (from within the past two years of employment).
  • License/certifications must be uploaded with profile (current NH license and BLS, 3 years of current experience).
  • Background issues/hits must be disclosed at time of profile upload.
Certification Requirements
  • BLS (AHA).
State License Requirements
  • New Hampshire.
Additional License Requirements
  • NH or Compact RN license must be in hand at submission.

This assignment location is 2 hours from Boston, MA.

SULLIVAN COUNTY WILL NOT CONSIDER ANY TRAVELER WHO RESIDES WITHIN 50 MILES OF FACILITY. To avoid delays in processing, please include perm address at profile upload.

Requested time off during the course of this assignment MUST be noted and verified at upload. RTO requests after upload will not be approved. NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).

Traveler must be willing to work ALL holidays, including weekend before (if needed/scheduled). RTO requests for holidays will not be approved.
