Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Long Term Care setting. As an LPN, you will be responsible for providing high-quality patient care, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Observe and assess residents' physical and mental well-being, reporting any changes or concerns to the supervisor.
  • Accurately transcribe and carry out physician's orders, administering medications and treatments as needed.
  • Supervise Licensed Nursing Assistants (LNAs) and ensure they are providing quality care, assigning and measuring nursing staff resident care consistent with their abilities and job description.
  • Participate in resident rounds with physicians to address medical needs and facilitate communication.
  • Administer oxygen and gastrostomy/jejunostomy feedings according to facility guidelines and procedures.
  • Perform wound care and have knowledge of wound vacs.
  • Complete 24-hour reports, check new orders, and maintain consistent communication with the Nurse Manager/Nursing Supervisor regarding resident issues or concerns.
  • Communicate with residents' families to resolve concerns and problems, notifying them of new orders and obtaining consent when necessary.
  • Assume responsibility for the management of assigned units during shifts under the direction of the Nursing Supervisor (NS) or Director of Nursing Services (DON).
  • Follow State and Federal Regulations regarding the storage, usage, and documentation of controlled medications.
Requirements
  • 3 years of current experience as an LPN (within 6 months) is non-negotiable.
  • Must be willing to float.
  • IV certification is preferred.
  • Previous travel experience and rural/small facility experience are highly preferred.

  • Skills checklist (pertinent to position) current to 1 year.
  • 1 supervisory signed reference (from within the past two years of employment).
  • License/certifications must be uploaded with profile (current NH license and BLS 3-year current experience).
  • Background issues/hits must be disclosed at time of profile upload.
Certification Requirements
  • BLS (AHA).
State License Requirements
  • New Hampshire.
Additional License Requirements
  • NH or Compact RN license must be in hand at submission. Facility will not pend.

This assignment location is 2 hours from Boston MA.
