In this role, you will oversee the anticoagulation clinic, assessing patients, initiating and monitoring treatment, and collaborating closely with the consultant haematologist.
Working Schedule: Three days a week, 7.5 hours each day.
Key Responsibilities- Lead specialized anticoagulation clinics, ensuring safe, effective, and economical use of anticoagulants to support patient admissions and maintain continuity of care between primary and secondary healthcare.
- Serve as a role model, delivering a consistently high standard of pharmaceutical care aligned with departmental and organizational goals.
- Educate healthcare professionals regarding the financial implications of medications within clinical haematology, particularly focusing on anticoagulant therapies, while advocating for evidence-based and cost-efficient prescribing practices.
- Contribute to the development of business cases to support advancements in Pharmacy, Clinical Haematology, and Anticoagulation services as required.
- Actively engage in the Thrombosis Committee and departmental meetings.
- Assist divisions in managing anticoagulant expenditures and serve as the liaison for service enhancements related to anticoagulation.
- Facilitate the education and training of other healthcare professionals.
- Participate in and contribute to clinical training programs at both local and national levels (e.g., summer students, pharmacy undergraduates).
- Engage in continuous professional development to keep knowledge current.

Job Duties- Engage in the frontline delivery of Pharmacy Services as needed.
- Act as a clinical role model for fellow pharmacists, demonstrating the ability to provide a service that guarantees the safe, clinically effective, and economical use of medications.
- Lead the provision of pharmacy-led anticoagulant clinics, coordinating training and evaluation for new pharmacists.
- Contribute to the formulation and execution of strategies aimed at enhancing safe medication use across the organization.
- Collaborate with primary care colleagues to address and communicate patient medication concerns to other healthcare professionals.
- Guide and promote the appropriate use of anticoagulants throughout the organization, including establishing and monitoring quality standards and providing feedback to senior pharmacy management.
- Represent pharmacy interests and lead service improvements within clinical haematology, ensuring alignment with organizational and departmental objectives.
- Provide performance and activity reports to senior management at designated intervals.
- Identify resources necessary for enhancing pharmacy services within the division.
- Support the development and adherence to local and national guidelines/policies to ensure safe and cost-effective prescribing of anticoagulants.
- Collaborate in creating Integrated Care Pathways and Patient Group Directions for clinical haematology and anticoagulation services.
- Work with senior pharmacy staff to ensure effective delivery of Pharmacy Services.
- Contribute to managing anticoagulant expenditures across divisions through monitoring, forecasting, and audits.
- Identify potential cost-saving opportunities for anticoagulants and implement strategies to achieve savings and control prescribing costs.
- MPharm or equivalent qualification.
- Registered with the General Pharmaceutical Council (GPhC).
- A minimum of three years post-registration experience as an independent prescriber, with specific experience in managing venous thromboembolism (mandatory).
This position is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, necessitating a Disclosure submission to the Disclosure and Barring Service to check for any prior criminal convictions.
