About the Role

We are seeking a skilled and compassionate Healthcare Support Worker to join our team at Hywel Dda University Health Board. As a key member of our Home First approach, you will play a vital role in providing short-term, intensive rehabilitation at home for patients to enable them to return home from hospital sooner.

Key Responsibilities
  • Deliver delegated care across a range of disciplines as part of a team-based approach to delivering holistic, patient-centred care.
  • Support people to achieve independence through the delivery of an individualised plan of care as determined by the team.
  • Communicate effectively with other team members at all times across all of the integrated teams and demonstrate a flexible approach to the needs of the service across the 7 days.
Requirements
  • NVQ Level 3 in Health and Social Care or working towards with equivalent level of experience.
  • Good literacy and numerical skills.
  • Experience of working in a Health or Social care environment.
