Position Overview

We are seeking a dedicated and skilled Senior Clinical Nurse Practitioner to apply a variety of advanced competencies and specialized knowledge for patients presenting with complex and undiagnosed health issues. In alignment with the government-led Primary Care Network (PCN) initiative, we are looking for a Senior Clinical Nurse Practitioner who embodies our values and is eager to provide outstanding care, joining our existing team of highly qualified professionals.

Key Responsibilities

This role will encompass the assessment and triage of patients, including same-day evaluations, and where appropriate, delivering definitive treatment (including medication prescriptions in accordance with established policies, patient group directives, and local clinical guidelines) or facilitating necessary referrals to other members of the primary care team.

The successful candidate will integrate into a dynamic and forward-thinking multi-disciplinary team within the PCN, which comprises two local GP practices.

We welcome applications from individuals seeking flexible working arrangements, such as annualized contracts or full-time hours condensed into 3-4 days.

Job Duties

The role involves:

  • Independently conducting clinical examinations and assessments of patients, initiating appropriate actions.
  • Operating at a master's level or equivalent, encompassing the four pillars of clinical practice, leadership and management, education, and research, while demonstrating core capabilities and specific clinical competencies.
  • Managing undifferentiated and undiagnosed conditions, identifying critical warning signs and serious underlying health issues, and taking appropriate actions.
  • Utilizing complex decision-making skills to inform diagnoses, investigations, and the comprehensive management of care episodes within a broad scope of practice, actively engaging in personalized and population-centered care approaches to facilitate shared decision-making with patients.
  • Completing relevant training to provide multi-professional clinical practice and supervision for other roles within primary care.

Essential Qualifications:

  • Meets DBS reference standards and maintains a clear criminal record, in accordance with legal requirements.
  • Ability to travel between practices in the locality.
  • Access to personal transport and capability to travel regularly across the locality, including home visits.

Qualifications

Essential:

  • Graduation from a Centre for Advancing Practice accredited MSc advanced practice program or completion of the Centre's Portfolio (supported) Route, both pathways leading to eligibility for an Advanced digital badge.
  • NMC Registered Nurse with clinical post-registration experience.
  • Compliance with NMC revalidation requirements.
  • Evidence of continuous professional development relevant to advanced nursing practice within the last two years.
  • Health assessment/minor illness qualification.

Desirable:

  • Driving license.
  • Completion of core mandatory training.

Experience

  • Experience within the NHS.
  • Experience in a GP practice setting.
