About the Role

We are seeking a highly skilled and experienced Senior Nurse Practitioner to lead our Rapid Response Service in Taunton and West Somerset. As a key member of our team, you will be responsible for providing clinical leadership and management to ensure the delivery of high-quality patient care.

Main Responsibilities
  • Provide clinical oversight and support to the team, ensuring that patients receive the best possible care and treatment.
  • Work closely with clinical supervisors and Band 6 Therapists to proactively support patients on the caseload, preventing unnecessary hospital admissions and promoting independent living.
  • Manage the day-to-day operational leadership of Rapid Response, ensuring services are delivered in accordance with agreed objectives, targets, and quality standards.
  • Collaborate with other agencies and healthcare professionals to meet the health and social needs of patients.
Requirements
  • Minimum 5 years of experience as a Band 6 healthcare professional.
  • Knowledge of the adult health and social care agenda.
  • Ability to work effectively across organisational boundaries and provide high-quality services within resource limits.
  • Non-medical prescribing qualification.
  • Valid driving licence and access to a car for business use.
