About the Role

We are seeking a highly skilled and experienced Senior Nurse Practitioner to join our team as a Service Lead for our Taunton and West Somerset Team in our Rapid Response Service.

Main Responsibilities
  • Provide clinical leadership and supervision to staff within the team, ensuring a high standard of care and treatment for patients.
  • Work closely with other members of the team and other agencies to best meet the patients' health and social needs.
  • Be responsible for the day-to-day operational leadership and management of Rapid Response, providing services to patients in accordance with agreed objectives, targets, quality standards, controls, and resource constraints.
  • Develop and implement action plans to meet specific national and local targets, and ensure the service is delivered within a performance management framework.

Requirements
  • Minimum 5 years Band 6 experience.
  • Experience of working across organisational boundaries in a healthcare setting.
  • Experience of providing high-quality effective services within resource limits.
  • Knowledge of the adult health and social care agenda.
  • Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance.
