The Role:
We're seeking a dedicated Support Worker to join our team and support individuals in achieving their goals and aspirations.
Key Responsibilities:
- To provide person-centred support to individuals, promoting their independence, choice, and control.
- To work collaboratively with colleagues and external agencies to ensure the best possible outcomes for individuals.
- To contribute to the development and review of individual support plans, ensuring they reflect changing needs and wishes.
- To support individuals to engage in their local community, promoting social inclusion and participation.
- To maintain accurate records and contribute to the evaluation of support services.
