About the Role

Southern Health NHS Foundation Trust is seeking a qualified Senior Mental Health Practitioner to join our team in Southampton Central Community Mental Health Team. As a member of our multi-disciplinary team, you will work with adults with severe or enduring mental health needs, providing care coordination and support to help them manage their symptoms and improve their overall well-being.

Main Responsibilities

  • Provide comprehensive care coordination and support to service users with severe or enduring mental health problems.
  • Assess and monitor service users' mental state, provide comprehensive risk assessment, and implement a management plan.
  • Deliver evidence-based interventions, provide advice and support on issues such as housing, benefits, education, employment, medication, and social activities.
  • Work collaboratively with service users as experts in their own care, promoting the principles of recovery, choice, and personalization.

Requirements

  • Qualified as a Registered Mental Health Nurse, Social Worker, or Occupational Therapist.
  • In-depth experience of working with adults with severe or enduring mental health problems.
  • Access to a vehicle for work purposes.
