Role Overview:
We have a full-time Lead Social Care Practitioner position available in our Hospital Discharge and Assessment team, located in Gloucester. The team benefits from a supportive and engaged management structure, which includes an Integrated Social Care Manager, Deputy Social Care Manager, Social Work Leads, Social Care Lead, Senior Social Worker, Social Workers, Social Care Practitioners, and an administrative team.
The Lead Social Care Practitioner role represents an excellent next step in your career; this position offers increased challenges and complexities without the additional management responsibilities. We rely on our Lead Social Care Practitioners to deliver expert social work practice in intricate situations, assessing and managing elevated levels of risk. You will collaborate closely with a variety of professionals and senior managers to ensure a balance of support and independence is achieved through expert, evidence-based opinions.

Hold a social work qualification and be registered with SWE* Have completed your ASYE* program and possess at least 2 years of post-qualification experience Thrive in a dynamic work environment Be passionate about promoting individuals' strengths and capable of making a difference through a safe approach to social work practice Maintain current knowledge of legislation and demonstrate experience in providing credible practice approaches.
