About the Role

We are seeking a highly skilled and dedicated Adult Social Care Practitioner to join our Gloucester Locality Team. As a key member of our team, you will be responsible for undertaking assessments and support plans with individuals who have physical disabilities or other complex needs.

You will work closely with families, carers, and other professionals, agencies, and organisations across Gloucestershire to promote independence and enable individuals to remain in their own homes. This role requires a strong understanding of the Care Act and the ability to apply it in practice.

About You

To be successful in this role, you will need to have relevant qualifications equivalent to NVQ Level 3 or equivalent health and social care experience. You should also have training related to health and social care assessments, experience of managing a caseload, and evidence of good interpersonal skills, verbal and written communication, and IT skills.
