Key Responsibilities
- Provide physiotherapy, occupational therapy, and rehabilitation services to clients in their homes and communities.
- Work collaboratively with clients, their families, and other healthcare professionals to develop and implement personalized care plans.
- Conduct thorough assessments and evaluations to identify client needs and develop effective treatment plans.
- Deliver evidence-based interventions and therapies to promote client independence, mobility, and overall well-being.
- Monitor client progress and adjust treatment plans as needed to ensure optimal outcomes.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
Requirements
