About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at HC-One Ltd. As a Care Coordinator, you will play a vital role in leading our care team to provide personalized care to our residents, exceeding their expectations and promoting a culture of kindness and respect.

With a strong background in care and leadership, you will be responsible for managing and developing our care team, ensuring that our residents receive the highest quality care and support. You will work closely with our residents, their families, and our care team to understand their needs and preferences, and develop care plans that meet their individual needs.

Key Responsibilities
  • Lead and manage the care team to provide high-quality care and support to residents
  • Develop and implement care plans that meet the individual needs of residents
  • Work closely with residents, their families, and the care team to understand their needs and preferences
  • Manage and develop the care team, providing coaching and training as needed
  • Ensure that our residents receive the highest quality care and support, exceeding their expectations
