About the Role

We are seeking a dedicated and compassionate Care Assistant to join our team at Connected Health. As a Care Assistant, you will play a vital role in providing high-quality care and support to individuals in their own homes.

Key Responsibilities
  • Provide personal care and daily living tasks to service users, as agreed with their families and professionals involved in their care plan.
  • Work within the policies and procedures of Connected Health, and if required, Next of Kin.
  • Develop and maintain professional working relationships with service users and colleagues from diverse backgrounds.
  • Communicate regularly with the immediate supervisor, particularly regarding changes in the service user's condition or circumstances.
  • Use the services on-call system in accordance with the service guidance.
  • Attend training, team meetings, and individual supervision sessions as required.
  • Complete documentation, including service user records and timesheets.
  • Comply with all Health and Safety Policies and Procedures.
  • Participate in supervision, staff meetings, and training activities as required.
  • Be responsible for maintaining and improving own knowledge and skills through experience and training.
Requirements
  • No formal experience is required, as full accredited training will be provided.
  • You must have an empathetic nature.
  • You must be a driver with access to a car.
