We are seeking a dedicated and compassionate Care and Support Specialist to join our team at Live Connected. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with mental health, learning disabilities, dementia, and complex needs.

Key Responsibilities:

  • Undertake daily living tasks as agreed with service users, their families, and professionals involved in their care plans.
  • Work within the policies and procedures of Live Connected, ensuring the highest standards of care and safety.
  • Complete health and safety checks within the service, adhering to our policies and procedures.
  • Lone work confidently, in line with our lone worker risk assessment.
  • Ensure safe medication delivery and ordering for service users.
  • Provide opportunities for service users to express their preferences and promote choice and independence.
  • Manage service users' behaviors in the least restrictive way possible.
  • Complete daily notes, handovers, and relevant documentation by the end of each shift, ensuring high-quality and efficient handovers.
  • Work collaboratively with the multidisciplinary team to support service users' emotional and physical well-being.
  • Develop and maintain professional relationships with service users and the MDT to ensure positive outcomes.
